We accept the following forms of Payments & Insurances

1. Private Pay (via Personal Check / Cashier's Check / Zelle Payments to 'christinasalh@gmail.com')
2. LTC: Long Term Care Private Insurance (Most Insurance Companies accepted)
3. Arizona Complete Health ALTCS (AHCCCS Medicaid): Starting 10/1/2024, we are contracted with them and can take residents who are planning on going to ALTCS in the near future (Medicaid Spend Down) or already are on ALTCS
4. Mercy Care ALTCS (AHCCCS Medicaid) - We are contracted with them and can take residents who are planning on going to ALTCS in the near future (Medicaid Spend Down) or already are on ALTCS

1. COVID-19 negative test
2. TB test (No signs/symptoms of TB)
3. Prescriptions as ordered by the Doctor (including OTC drugs) / Part of 'Physician's Paperwork'
4. POA copy (when applicable)
5. Social Security/Pension/Benefits statement (when applicable)
